Position Description

Join Harbor Health Services, Inc. as we grow our newest community health center in historic Plymouth, MA!

The Nurse Practitioner provides comprehensive outpatient care to a panel of patients including routine health maintenance, management of chronic conditions, management of acute illness, and referral for specialty care. The Nurse Practitioner shares responsibilities and collaborates with Physician in the care of patients within our Patient Centered Medical Home care model. 

Minimum Skills, Experience and Educational Requirements:
Current Massachusetts NP License, strongly desire candidate with 2 + years of experience in an ambulatory setting.

Required Certifications and Licenses:

  • Certification eligible as a Family Medicine NP,
  • Current DEA, Mass Controlled Substance Registration and CPR Certification.

Harbor Community Health Center-Plymouth is a Federally Qualified Health Center and employed providers are eligible to participate in Federal and State Loan Repayment Programs.
