Position Description

Primary responsibilities include management of patients and program coordination at an accredited Cystic Fibrosis center. The center has separate pediatric and adult programs, with about 55-60 patients in each program. The coordinator is expected to provide care for ambulatory and hospitalized patients with MD supervisor as needed/required in RI; manage phone calls and all clinical activities related to CF care. The coordinator is also expected to participate in quality improvement activities in both pediatric and adult programs, especially as they relate to implementation of care guidelines.  Research opportunities may also be available.

Responsibilities:

  • Demonstrates a fundamental grounding in nursing theory and practice with mastery of knowledge within a defined content area.
  • Manages a caseload of patients in partnership with the healthcare team to optimize health status and coordinate care over time and across healthcare settings.
  • Provides leadership in the development, validation, implementation, and evaluation of standards, policies, procedures, and critical paths to support the patient care delivery model.
  • Oversees care coordination in acute, home care, and ambulatory care settings as feasible.
  • Assists the healthcare team to achieve mutually agreed upon healthcare goals by providing necessary support, education and encouragement.
  • Demonstrates advanced physical and psychosocial assessment skills and knowledge of family dynamics.
  • Provides direct care to patients in acute care, ambulatory care and home care settings.
  • Performs advanced practice skills under the supervision of the Medical Director.
  • Provides expert consultation for clinical issues promoting increased knowledge and skills for the consultee.
  • Participates in the education, clinical research and mentoring of healthcare providers.
  • Facilitates patient/family education in area of expertise. Annually completes mandatory safety infection control, BLS, and ACLS/PALS. Prescriptive privileges per licensure.

Other information:

  • Masters of Science Degree, Graduate of accredited Nurse Practitioner program.  
  • Baccalaureate in Nursing
  • Certified in Basic Cardiac Life Support (BLS).
  • Demonstrates knowledge and skills necessary to provide care to patients with consideration of aging processes, human development stages and cultural patterns in each step of the care process.

Experience:
Two years related experience as RN in acute care hospital setting and preferred 1-2 years experience as a nurse practitioner.  Family NP training is preferred, but pediatric and adult trained practitioners will be considered.
